What We Collect
To provide dental care we collect: contact details, health and dental history, treatment records including x-rays and photographs, insurance and billing information, and appointment communications. Website forms collect only what you type into them; drafts autosave to your own device (browser storage), not our servers, until you press submit.
How We Use It
Your information is used to provide and document your dental care, communicate about appointments and recalls, process payments and insurance claims (including direct billing and the CDCP), and meet our legal and regulatory obligations as a dental practice in Ontario under PHIPA (the Personal Health Information Protection Act).
Who We Share It With
Only those involved in your care and billing: your insurer (for claims you ask us to submit), dental laboratories and specialists involved in your treatment, and regulators where legally required. We never sell your information, and we don't use your health information for marketing without consent.
Your Rights
You may access your records, request corrections, and withdraw consents (like reminder communications) at any time by contacting the office. Copies of records for transfer to another dentist may involve a reasonable administrative fee.
Safeguards
Records are protected through physical, administrative and technical safeguards, and our team is trained on privacy obligations. Questions or concerns?
